How to Keep Your Coverage

As a Healthy Families Program member, you'll need to renew your health coverage every year.

There are 3 easy steps to renew your Healthy Families Program membership:

1 

Watch for a letter and renewal form. It will come to you in the mail from The Healthy Families Program.
2 Fill out all the renewal form and return it as soon as possible. It's very important that you return it by the due date, which should be listed in the letter or on the form.
3 If you move, be sure to call the Healthy Families Program right away at (800)880-5305.  

Your Healthy Families Program coverage expires one year after you enroll in the program.  If you don't get your renewal forms in mail at least 60-days from when your health insurance will expire, call the Healthy Families Program right away at (800)880-5305 and ask that your renewal forms be sent to you.

If you did receive your renewal packet and have questions about how to fill out the forms, contact our Enrollment Team.  Be sure to include your name, phone number, and a explanation of how we can help you.

We will help you fill out the renewal form over the phone, and even make an appointment for you to come to our office to fill out the form.
