Get a new ID Card

San Francisco Health Plan issues each Member an ID Card. When you get your Member ID Card, check all information to make sure it is correct. You may have a new ID Card mailed to you at no charge if:

  • Any information is incorrect;
  • Your card is lost or stolen;
  • You change your primary care provider:
  • Your address or other information changes.
