How do we measure it?

Improving Childhood Immunizations

 Childhood Immunization
 

Measures the percentage of children who recieve the following shots by their second birthday:

  • 4 diphtheria, tetanus and acellular pertussis (DTaP)
  • 3 polio (IPV)
  • 1 measles, mumps and rubella (MMR)
  • 3 H influenza type B (HiB)
  • 3 Hepatitis B
  • 1chicken pox (VZV)
  • 2 hepatitis A
  • 3 rotavirus (or 2 Rotarix)
  • 4 pneumococcal conjugate vaccines

Improving Well Child Visits

 Well Child

 

Measures the percentage of enrolled members 3–6 years of age who received one or more well-child visits during the measurement year

Improving Diabetes Care

 

    

 

 

 

Measures the percentage of members 18-75 years of age with diabetes (type 1 or type 2) who had each of the following:

  • Hemoglobin A1c (HbA1c) testing
  • HbA1c poor control (>9.0%)
  • LDL-C screening
  • LDL-C control (<100mg/dL)
  • Eye exam (retinal) performed
  • Medical attention for nephropathy
  • Blood pressure control (<140/90) 

Improving Prenatal Care

 

Measures the percentage pregnant members who have had their prenatal checkup within timeframe - 42 days for new members and 1st trimester for existing members

Improving Postpartum Care
Measures the percentage of members who receive a maternal health visit between 3 and 8 weeks postpartum
