Improving Childhood Immunizations	Member reminder card (Chinese, English, Spanish) Member incentive card (Chinese, English, Spanish, Form Only) Automated phone call as reminder	Measures the percentage of children who recieve the following shots by their second birthday: 4 diphtheria, tetanus and acellular pertussis (DTaP) 3 polio (IPV) 1 measles, mumps and rubella (MMR) 3 H influenza type B (HiB) 3 Hepatitis B 1chicken pox (VZV) 2 hepatitis A 3 rotavirus (or 2 Rotarix) 4 pneumococcal conjugate vaccines
Improving Well Child Visits	Member incentive card (Chinese, English, Spanish, Form Only) Automated phone call as reminder	Measures the percentage of enrolled members 3–6 years of age who received one or more well-child visits during the measurement year
Improving Diabetes Care	Member incentive (Chinese, English, Russian, Spanish, Vietnamese)	Measures the percentage of members 18-75 years of age with diabetes (type 1 or type 2) who had each of the following: Hemoglobin A1c (HbA1c) testing HbA1c poor control (>9.0%) LDL-C screening LDL-C control (<100mg/dL) Eye exam (retinal) performed Medical attention for nephropathy Blood pressure control (<140/90)
Improving Prenatal Care	Member incentive card (Chinese, English, Spanish, Vietnamese, Form Only)	Measures the percentage pregnant members who have had their prenatal checkup within timeframe - 42 days for new members and 1st trimester for existing members
Improving Postpartum Care	Member incentive card (Chinese, English, Spanish, Vietnamese)	Measures the percentage of members who receive a maternal health visit between 3 and 8 weeks postpartum
