Strength in Numbers

The Strength in Numbers Program (SI#s) aims to improve chronic care and patient experience through shared data reporting, technical assistance, panel management training, and modest incentives for improvements.  SI#s engages primary care clinics in the SFHP and Healthy San Francisco (HSF) provider networks to share data and improve performance on key population health and access measures.

Through this program, participating clinics receive financial incentives linked to performance quality, access and meaningful use measures.  SFHP and HSF also provide technical assistance to clinic staff to support population management activities. Technical assistance training topics in 2011 included panel management using electronic registries, and onsite mentoring for clinic staff to improve use of “in-reach” activities to ensure patients receive the right care at the right time, whatever the reason for their appointment.
