Find a Pharmacy

Healthy Workers members must have prescriptions filled at either:

  • The San Francisco General Hospital Outpatient Pharmacy:
    1001 Potrero Avenue, San Francisco
  • The pharmacy used by your clinic

FOLLOW THESE STEPS:

  • To find your clinic, refer to the back of your San Francisco Health Plan ID Card.
  • Locate your clinic in the chart below to find your new pharmacy
  • Call or visit your clinic to make sure you are registered.
  • If you currently get medicine refilled at a different pharmacy, you must contact your doctor for a new prescription.
