Each member of Healthy Workers HMO must choose a primary care provider (PCP) or a primary care clinic. The online Provider Search Tool helps you find and choose the right PCP for you and your family. Please pick your PCP or clinic from the Provider Search Tool. PCPs and clinics are listed by neighborhood. The PCP or clinic you choose will oversee all of your health care needs. If you need specialty care, your PCP or clinic will help you find other health care providers, called specialists, and will refer you to them when needed.

Use of SFHP Logos, Names and Service Marks:
No external organization, provider or vendor may use the San Francisco Health Plan (SFHP) logo, name, service mark, trademark, or likeness without obtaining prior written consent from SFHP and agreement to comply with any applicable conditions for use.   Such consent should be procured at least 90 days in advance of proposed usage.
