Your PCP is your partner in your health journey

You can improve your health by visiting your primary care provider (PCP). Your PCP is the main doctor, physician assistant, or nurse practitioner in charge of your health care. A wellness check-up with your PCP is important for kids, pregnant people, and people with long-term health problems, like asthma or diabetes.

When you join SFHP, you should visit your PCP for a wellness check-up even if you are healthy.

Getting a wellness check with your PCP can help you to:

  • Live longer
  • Lower your risk for health problems
  • Keep you fit and active
  • Find illnesses early
  • Prevent infections like the flu
  • Get support with food, housing, transportation, and more
  • Manage health conditions
  • Have a better quality of life

During a wellness check:

  • Your vital signs will be checked (such as weight and blood pressure).
  • Your PCP will ask about you and your family’s health.
  • You can ask any questions or share concerns about your physical or mental health.
  • Your PCP can connect you to a health class or help with food, housing, transportation, and other support.

Get Help in Your Language

Some cultures may have different ways of taking care of health needs. Some people use natural remedies while others visit a doctor often. It is okay to feel nervous if going to a doctor is new for you. Your PCP is there to help you!

  • You can get a PCP who speaks your language.
  • You can also ask to have an interpreter. The interpreter will be with you in-person or on the phone during your PCP appointment. Learn more at Language Interpreter Services.

Make an appointment for a wellness check-up by calling your PCP. You can find your PCP’s phone number on your SFHP ID card. You can meet with your PCP in-person or online by phone or video. If you need help making an appointment, you can call SFHP Customer Service.
