Support for Older Adults and Adults with Disabilities

Community-Based Adult Services (CBAS) is a daytime health program for older adults and adults with disabilities. CBAS helps you keep your independence and live on your own terms. You can go to a CBAS Center at no-cost to prevent going to the hospital in the future or being put into a nursing home.

At a CBAS Center, you can get help with:

  • Self-care skills
  • Health and wellness check-ups
  • Talking with your Primary Care Provider* (PCP)
  • Tracking your medicine
  • Getting healthy foods
  • Checking your mental health
  • Social and physical activities like group walks
  • Testing for COVID-19

*Your Primary Care Provider (PCP) is your doctor, physician assistant, or nurse practitioner in charge of your health care.

You can go to a CBAS Center if you are 18 years or older and tend to:

  • Be in poor health.
  • Have many emergency room visits or hospital stays.
  • Need support for daily activities.
  • Have no family/caregiver or are often alone.
  • Get confused or don’t know where you are.
  • Wander or get lost.
