Keeping You Healthy
As an SFHP member, you have access to services that can help keep you healthy. These services are meant to find health problems early and keep them from getting worse.
Why Preventive Care is Important
Preventive care helps you stay healthy. It can help you avoid getting sick with serious health problems like diabetes, heart disease, and cancer. It can also help you manage health problems you already have.
Services Covered by SFHP
SFHP covers many preventive services at no cost to you. These services include:
Behavioral Health: Behavioral health care like counseling can help you make healthy choices, like quitting smoking or managing your weight.
Check-ups: Regular check-ups with your doctor can help you stay healthy and talk about any health concerns you have.
Community Health Worker Services: Get support from community health workers who can help you navigate your health care and connect you with resources.
Dental Care: Keep your teeth and gums healthy with dental check-ups and cleanings.
Health Education & Classes: Learn how to stay healthy with classes and education programs. Topics may include healthy eating, exercise, and managing stress.

Screenings: Screenings can find and treat health problems early. You can get screened for diabetes, high blood pressure, sexually transmitted infections (STIs), and more.
Sensitive Care: Get care for sensitive health issues in a safe and supportive place. Topics may include pregnancy, STI/HIV prevention, treatment for substance use, and more. If you are under age 18, you can get certain services without permission from a parent or guardian.

Vision Care: Take care of your eyes with vision screenings and eye exams.
Well-child Visits: Well-child visits are important for kids. They help make sure kids are growing and developing as they should.

How to Get Preventive Care
Getting preventive care with SFHP is easy. Just call your primary care provider (PCP) to make an appointment. A PCP is your main doctor, physician assistant, or nurse practitioner. You can find your PCP’s phone number on your SFHP ID card. Or, you can find a PCP near you with the Provider Directory.
Many preventive services can be done during your regular visit, so you don’t need a separate appointment. Ask your PCP what vaccines and tests may be right for you.
