Vision Services

If you are an SFHP Healthy Workers HMO member, you can get vision services from the Vision Service Plan (VSP).

Your coverage with a VSP provider includes:

WellVision Exams

You can get an eye exam every calendar year to help you see well and to check for other issues like diabetes and high blood pressure. Your copay is $10.

Prescription Glasses

Frames
Every other calendar year, you can get:

• $120 allowance to use for featured frame brands.
• $100 regular frame allowance.
• 20% savings on the amount over your frame allowance.
• $55 Costco frame allowance.
• Your copay is $25.

Lenses
Every other calendar year, you can get:

• Single vision lined bifocal and lined trifocal lenses.
• Impact-resistant lenses for dependent children.
• Your copay is $25.

Lens Enhancements

• Premium progressive lenses. Copay of $95 – $105.
• Custom progressive lenses. Copay of $150 – $175.
• Average savings of 20-25% on other lens enhancements.

Contact Lenses
Every other calendar year, instead of glasses, you can get:

• $100 allowance for contacts and contact lens exam (fitting and evaluation).
• 15% savings on a contact lens exam (fitting and evaluation).
• Your copay is $0.

More Services

You can get laser vision correction at an average of 15% off the regular price, with discounts available at contracted facilities.

You can also get 20% savings on unlimited additional pairs of prescription or non-prescription glasses/sunglasses, including lens enhancements, from a VSP provider within 12 months of your last exam. You can find more eyewear offers and savings at vsp.com/offers.

If you are a San Francisco Health Plan Healthy Workers HMO member, then you receive vision services through Vision Service Plan (VSP).

To find a VSP provider, visit vsp.com, or call 1(800) 438-4560 and ask for help finding a doctor in the “Advantage” Network. VSP will give you contact information for a participating provider.

When you call the participating provider, tell them you are a VSP member. The provider will check your plan coverage. If you are eligible for services, your provider will make an appointment for you. If you are not eligible for services, the provider will tell you. VSP will pay the provider directly for covered services and materials.
