Eligibility
To join San Francisco Health Plan, you must be eligible and enrolled in one of our health coverage programs and in addition be living in San Francisco. Each program has its own eligibility guidelines. Which program you may be eligible for depends on household income, family size, and age.

How to Apply
The first step to enrolling with San Francisco Health Plan is to see if you or your child qualifies for any of our programs. To figure out which program is right, we’ll ask a few questions about family income, age, San Francisco residency and in some cases student or parental status.
The SFHP Service Center is a place where you can go to get information on health care access options that are available for you and your family. Our Enrollment Team is available, by appointment, to provide application assistance for the Medi-Cal, Healthy San Francisco, and San Francisco City Option Programs. You or your child may qualify for one of our programs if you fit into one of the following categories:

Asking a member of our Enrollment Team is the best way to know for sure if you qualify for one of our health care coverage programs.
Starting June 6th, 2022, the SFHP Service Center will be open three days a week for in-person application and enrollment services. Appointments are now available both in-person and by phone.
